Child Information

  • In consideration for my child(ren)’s participation in this school, I waive and release all claims of injuries my child(ren) may sustain during the course of this program. As the parent/guardian of the child, I recognize and acknowledge that there are certain inherent risks of injury. I agree to assume the full risk of any injuries, all damages or loss, including death, which may result from the child’s participation in activities connected with the school’s program.
  • In fulfillment of my above consideration, I waive all legal liability claims against Noor Al-Huda Weekend Islamic School, the Islamic Center of Peoria (ICP), and all of the two institution’s officers, agents, members, and volunteers.
  • For the safety of my child(ren), I will not leave my child unattended outside the ICP building before and after school hours. At the end of the school day, I will pick up my child on time because I understand that ICP cannot supervise children for an unreasonable time after school has ended.
